From Monday the 16th of October, Nightingale medical centre will be moving to a total triage system. This means, to access an appointment you will need to fill in a triage request. Once the request has been triaged we will contact you with the outcome, this may be an appointment with a clinician or it may be for further tests, information or signposting to a local service.
The triage request can be done via the link below
You can telephone and one of our operators can talk you through the form and fill in for you. This can also be filled in for admin requests such as sick notes, letters, referral updates, medication and information about test results.
We will only have a set number of medical triage requests per day, so that we can safely treat our patients. After this, you will need to contact 111 who can assess you and inform us if they feel you need further input from us.
We have moved to this format to address the increase in demand the surgery has seen over the last few years but also to ensure we are seeing the patients who are in most need, and dealing with any medical issues as quickly and safely as we can.
